Privacy

 

Jouw privacy is natuurlijk erg belangrijk voor ons. Wij doen er dan ook alles aan om deze goed te beschermen tegen derden.

De gegevens van jou die wij registeren in jouw dossier die nodig zijn voor het verlenen van hulp en het vergoed krijgen van die hulp, worden geregistreerd volgens de Algemene Verordening Gegevensbescherming (AVG) en volgens de Wet Geneeskundige Behandelingsovereenkomst (WGBO). Iedere organisatie die met privacygevoelige informatie werkt is verplicht daar correct mee om te gaan. Jouw gegevens zijn dan ook niet toegankelijk voor derden en worden alleen gebruikt voor de behandeling en de vergoeding van de behandeling.

Ook geven wij nooit gegevens door aan derden als de huisarts, verzekeringsmaatschappij, UWV, arbo-artsen, advocaten en dergelijke zonder dat je ons daar voor machtigt. 

Jouw gegevens worden bij ons opgeslagen in een electronisch dossier. Wij maken hiervoor gebruik van Praktijkdata en zij zien er op toe dat uw dossier streng beveiligd is en niet toegankelijk is voor derden.

Uiteraard heb je zelf inzage in jouw dossier. Voor enkele gegevens kan dit tijdens een bestaand consult. Gaat dit om het gehele dossier dan kun je hiervoor een afspraak inplannen met de behandelaar. 

Mocht je het verzoek hebben om informatie te delen met een derde partij dan hebben wij daar een schriftelijke machtiging van jou voor nodig. De gedeelde informatie wordt dan ook vooraf nog met je doorgenomen en wij vragen jouw uitdrukkelijke toestemming dan om deze informatie te delen. 

Ook is het gebruikelijk om jouw verwijzer op de hoogte te stellen van jouw behandeling bij ons via een aanmeldbrief en een afsluitbrief. Ook hier vragen wij uitdrukkelijk jouw toestemming voor. Wil je dit niet dan doen wij dit ook niet.

Your privacy is extremely important to us. We therefore take every possible measure to ensure that your personal information is carefully protected and kept confidential.

The information we record in your client file, which is necessary for providing treatment and obtaining reimbursement for that treatment, is processed in accordance with the General Data Protection Regulation (GDPR) and the Dutch Medical Treatment Contracts Act (WGBO).

Any organization that works with sensitive personal information is legally required to handle such data with great care. Your personal information is therefore not accessible to unauthorized third parties and is used solely for the purpose of your treatment and, where applicable, reimbursement of treatment costs.

We never share information with third parties such as your General Practitioner (GP), health insurance company, occupational health physician, UWV, lawyer, or any other party without your explicit consent.

Your information is stored in a secure electronic health record. We use Praktijkdata for this purpose. Praktijkdata ensures that your file is securely protected and inaccessible to unauthorized third parties.

Of course, you have the right to access your own records. Access to specific information can often be discussed during a regular consultation. If you wish to review your complete file, an appointment can be arranged with your treating psychologist.

Should you request that information be shared with a third party, we will require your written authorization. Any information to be shared will first be discussed with you, and we will obtain your explicit consent before releasing it.

It is also common practice to keep your referring physician informed about your treatment through an initial referral letter and a discharge letter upon completion of treatment. We will always ask for your explicit permission before sending this information. If you do not wish us to do so, we will fully respect your decision and no information will be shared.
